It’s National Gum Care Month. Let’s a moment to talk about why it’s so important to take care of your gums.
Gum disease affects almost half of adults over age 30 and approximately 70 percent of adults over age 65. The first stage of gum disease is called gingivitis, an inflammation of the gums. With gingivitis, gums can be red and puffy, and bleed easily when brushing or flossing. If gingivitis is not treated, it can progress to periodontitis, where the structures supporting the teeth, including the bone, begin to break down and be lost. Advanced stages of gum disease can lead to tooth loss and general health problems.
The good news is that gum disease is treatable — and early gum disease is even reversible. So what can you do to take care of your gums?
- Be diligent about your oral hygiene routine at home: Your first line of defense is your oral hygiene routine at home. Brush your teeth gently morning and night, using a soft toothbrush and fluoride toothpaste. Brushing too vigorously can harm your gums and cause them to recede. It is also important to floss every day to dislodge plaque that can build up between the teeth and around the gum line.
- Come in for professional dental cleanings and exams: Schedule regular professional cleanings to remove the plaque that is hard to reach. If plaque is not removed, it can harden to form tartar (or “calculus”). Only professional cleanings with special dental tools can remove tartar. When plaque and tartar form below the gum line, your bone that supports the teeth may be at risk. We can examine your mouth above and below the gum line to detect and monitor any signs of gum disease and recommend appropriate treatments.

Archeologists can tell us quite a bit about our primitive ancestors. For example, because of their coarse, abrasive diet and a primitive understanding of oral hygiene, their teeth had a rough go of it. They simply wore out faster — a contributing factor, no doubt, to their short life spans of thirty or forty years.
But thanks to improvements in lifestyle, healthcare and diet, people live much longer today. And so do their teeth, thanks to advances in dental care and disease prevention. While teeth still wear to some degree as we age, if we care for them properly with daily oral hygiene and regular dental visits, we can keep that wear to a minimum. Teeth truly can last a lifetime.
Unfortunately, it's still all too common for people to lose their teeth prematurely. The main reason: the two most prevalent dental diseases, tooth decay and periodontal (gum) disease. Tooth decay arises from high concentrations of mouth acid that erode enamel, teeth's irreplaceable protective shell. Gum disease is an infection that damages the bone supporting tissues as it infiltrates deep below the visible gum line.
While they occur by different mechanisms, the two diseases have some commonalities. They both, of course, can lead to tooth loss. And, they're both triggered by oral bacteria found in dental plaque, a thin film of food particles built up on tooth and gum surfaces. Multiplying bacteria feed on plaque and produce acid as a by-product. And certain bacterial strains infect gum tissues.
Both of these diseases can be treated successfully, especially if detected early. But the better approach is to prevent them in the first place. This introduces another commonality — they share the same prevention strategy of daily, comprehensive brushing and flossing for plaque removal, regular dental cleanings and checkups, and a sharp eye for any signs of disease like bleeding gums or tooth pain.
With diligent dental care and close attention to your oral health, you increase your chances of avoiding the full threat of these diseases.Â And with healthy teeth, you have a better chance of living a long and healthy life.

Periodontal (gum) disease is potentially devastating to your teeth, gums and bone. To fight it we have to remove the substance that causes and sustains the disease from all oral surfaces — a thin layer of bacteria and food particles known as plaque.
To accomplish this task, we use a variety of hand instruments called scalers to mechanically remove plaque and calculus (hardened plaque deposits), as well as ultrasonic equipment to vibrate plaque loose and flush it away with water. If we detect plaque deposits well below the gum line and around the tooth roots, we may need to use other techniques like root planing or surgery to access these deeper areas.
Â While gum disease is persistent and aggressive, these traditional techniques have proven quite effective in controlling the infection and restoring health to diseased gums. Yet like other aspects of medicine and dentistry, technological advances have created a new option for gum disease treatment: the Nd:YAG laser.
The Nd:YAG laser is named for the crystal it uses to produce a narrow and intense beam of light on a specific frequency. In recent years it's become an important surgical tool because it can distinguish between diseased and healthy tissue, destroying the former while not affecting the latter.Â It's being used now on a limited basis for treating gum disease, especially for removing infected tissue in deep pockets that can form below the gum line, and for removing plaque and calculus from root surfaces.
Â Because of its precision, early evidence of effectiveness is encouraging: minimal tissue damage and swelling, less bleeding and reduced patient discomfort after treatment. The heat from the laser has also been shown to kill bacteria and essentially sterilize the area.
Still, the findings aren't conclusive enough as to whether lasers are superior in most circumstances to traditional scaling methods. For the time being, we'll continue to use the tried and true methods for removing plaque and calculus. But as laser technology advances, the time may come when this new approach to gum disease treatment will become a more prominent and beneficial option for patients.

Is there a link between periodontal (gum) disease and cardiovascular disease? Medical researchers are endeavoring to answer this intriguing question, but early findings seem to say yes. If it bears true, the findings could advance treatment for both diseases.
There is one thing that can be said for certain: inflammation is a factor in both diseases’ progression. Gum disease begins as an infection caused by bacteria growing in plaque, which is made up of bacteria and a thin film of food remnant that adheres to tooth surfaces. The body responds to this infection through tissue inflammation, an attempt to prevent the infection from spreading. Likewise, inflammation appears to be a similar response to changes in blood vessels afflicted by cardiovascular disease.
While inflammation is part of the body’s mechanism to heal traumatized tissue, if it becomes chronic it can actually have a damaging effect on the tissues intended to benefit. For patients with gum disease, chronic inflammation causes connective tissues to detach from teeth, leading eventually to tooth and bone loss. Similarly, inflammation damages the linings of blood vessels in cardiovascular disease patients.
Researchers want to know what role bacteria may also play in the progression of cardiovascular disease. Initial studies seem to indicate that proactively treating the gum disease by removing all plaque from oral surfaces in patients with both conditions does appear to improve the health of diseased blood vessel linings. Whether this could ultimately reduce the occurrence of heart attack or stroke still needs to be ascertained.
As we learn more about the possible connections between these two diseases, there’s hope it will lead to new advancements that could improve health outcomes for both. It may prove to be the case, then, that maintaining a healthy mouth promotes a healthy heart, and vice-versa.

If you have periodontal (gum) disease, you’ve no doubt experienced red and swollen gums. If, however, you notice an especially inflamed area next to a tooth, you may have developed a gum abscess.
An abscess is a pus-filled sac that develops as a result of chronic (long-standing) gum disease, an infection caused by bacterial plaque that’s built up on tooth surfaces from inadequate oral hygiene or from a foreign body (food debris) getting stuck below the gums. The abscess, which typically develops between the tooth and gums, may be accompanied by pain but not always (the affected tooth may also be tender to bite on). Abscesses may grow larger, precipitated by stress or by a general infection like a common cold, and then abate for a time.
As with other abscesses in the body, a gum abscess is treated by relieving the pressure (after numbing the area with local anesthesia) and allowing it to drain. This is often followed by cleaning any infected root surfaces of bacterial plaque and then irrigating the area with a saline and/or antibacterial solution. We may also prescribe antibiotics afterward and some form of pain control (usually a non-steroidal anti-inflammatory drug like ibuprofen) to help with discomfort.
Although the results of this procedure can be dramatic, it’s just the first step in treating the overall gum disease. After a few days of healing, we continue with a complete examination and recommend further treatment, usually starting with removing bacterial plaque and calculus (hardened plaque deposits), the underlying cause for the infection and inflammation, from all tooth and gum surfaces. This may take several sessions before we begin seeing the gum tissues return to a healthier state.
The key to preventing an abscess recurrence (or any symptom of gum disease) is to remove plaque everyday through proper brushing and flossing, and visiting us twice a year (or more if you’ve developed chronic gum disease) for cleanings and checkups. Doing so will raise your chances of avoiding an uncomfortable and often painful gum abscess in the future.
